We will need to collect as much information as possible. You will be sent a questionnaire before your first session. This gives us some background information so that we can set off on the right lines. The questionnaire includes contact details for you and your child, as well as some questions which support us in building a picture of your child, such as information about their interests, medical history and speech and language skills. We will also need signed consent from you so that we can see your child.
It is very helpful to share any other important information you have about your child as this helps us plan our assessment and decide appropriate next steps. Information that is useful to share includes Education Health Care Plans (EHCP), information from previous Speech and Language Therapy intervention, relevant information from other professionals and school records such as annual reviews and Individual Education Plans.
